U trenutku ozljede uobi\u010dajeno je primijetiti dinami\u010dki valgus koljena (koljenje koje se uru\u0161ava prema unutra), ograni\u010denu fleksiju koljena, lateralni i rotirani polo\u017eaj trupa te kontakt stopala s tlom.<\/p>\n\n\n\n<p class=\"\">Na koljenu, vr\u0161no naprezanje prednjeg kri\u017enog ligamenta (ACL) javlja se kao rezultat vi\u0161eravninskih sila, uklju\u010duju\u0107i silu prednje tibijalne ladice, valgus koljena i momente unutarnje rotacije tibije. Me\u0111utim, snaga i biomehanika su uvelike promjenjivi atributi.<\/strong><\/p>\n<\/blockquote>\n\n\n\n<p class=\"\">U gotovo svakom sportu \u2013 od amaterskih do elitnih razina natjecanja \u2013 djevojke i \u017eene imaju smanjen pristup trena\u017enim objektima, resursima (uklju\u010duju\u0107i treniranje) i medicinskoj skrbi u usporedbi s mu\u0161kim kolegama. \u0160tovi\u0161e, \u017eene su nedovoljno zastupljene u istra\u017eivanjima sporta i medicine vje\u017ebanja, \u0161to ograni\u010dava na\u0161e razumijevanje predisponiraju\u0107ih \u010dimbenika rizika i ote\u017eava sposobnost trenera i prakti\u010dara da provedu pristup treningu i rehabilitaciji utemeljen na dokazima.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Mo\u017eemo li sprije\u010diti ozljedu prednjeg kri\u017enog ukr\u0161tenog kri\u017ea (ACL)?<\/h2>\n\n\n\n<p class=\"\">Meta-analiza meta-analiza (meta-meta-analiza?) otkrila je da programi prevencije ozljeda ACL-a smanjuju rizik od beskontaktnih ozljeda ACL-a za dvije tre\u0107ine kod sporta\u0161ica. Kod nogometa\u0161ica, konkretno, postoje dokazi da vi\u0161ekomponentni \u201eneuromuskularni\u201c programi vje\u017ebanja smanjuju stopu ozljeda ACL-a za 45%. Me\u0111utim, pridr\u017eavanje ovih programa je slabo, a stope ozljeda pokazale su ograni\u010dene dokaze o padu izvan ovih kontroliranih studija. <\/p>\n\n\n\n<p class=\"\">Mo\u017eda programi treninga koji su u\u010dinkoviti kod sporta\u0161a ni\u017ee razine nisu u\u010dinkoviti ili izvedivi u slo\u017eenijem svijetu vrhunskog sporta? <\/p>\n\n\n\n<p class=\"\">Na primjer, manje od 1 od 3 ovih programa ispunjava smjernice za snagu, a 67% ispunjava smjernice za pliometrijski\/sna\u017eni trening. Tako\u0111er se mo\u017ee tvrditi da na\u0161e pobolj\u0161ano razumijevanje prevencije ozljeda ne uspijeva pratiti pove\u0107ane zahtjeve treninga i natjecanja.<\/p>\n\n\n\n<p class=\"\">Me\u0111utim, u na\u0161em nedavnom sustavnom pregledu (Lima, u pregledu), pokazujemo da programi prevencije ozljeda ACL-a tako\u0111er ne uspijevaju promijeniti biomehaniku donjih udova tijekom zadataka visokog rizika, \u0161to postavlja pitanje: koji je mehanizam(i) kojima sprje\u010davaju ozljede ACL-a?<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Primjena tehnologije temeljene na terenu za identifikaciju sporta\u0161ica s rizikom od ozljede prednjeg kri\u017enog ligamenta<\/h2>\n\n\n\n<p class=\"\">Razumijevanje \u010dimbenika koji pove\u0107avaju rizik od ozljeda sporta\u0161a predstavlja prvi korak prema razvoju ciljanih intervencija za sprje\u010davanje ozljeda. Do nedavno, nijedna studija nije istra\u017eivala \u010dimbenike rizika za ozljede ACL-a kod elitnih australskih nogometa\u0161ica ili nogometa\u0161ica - od kojih se \u010dini da prve imaju najve\u0107e stope ozljeda ACL-a od bilo kojeg sporta u svijetu.<\/p>\n\n\n\n<p class=\"\">Koriste\u0107i 10-minutnu bateriju terenskih testiranja u predsezoni, testirali smo izometrijsku snagu abduktora i aduktora kuka, snagu ekscentri\u010dnog fleksora koljena, kinetiku bilateralnog skoka u protupokretu (CMJ) i kinematiku trostrukog vertikalnog skoka na jednoj nozi koriste\u0107i VALD-ove metode kod 322 elitne nogometa\u0161ice. <a href=\"https:\/\/valdhealth.com\/products\/forceframe\" target=\"_blank\" rel=\"noreferrer noopener\">ForceFrame<\/a>, <a href=\"https:\/\/valdhealth.com\/products\/nordbord\" target=\"_blank\" rel=\"noreferrer noopener\">NordBord<\/a>, <a href=\"https:\/\/valdhealth.com\/products\/forcedecks\" target=\"_blank\" rel=\"noreferrer noopener\">ForceDecks<\/a> i <a href=\"https:\/\/valdhealth.com\/products\/humantrak\" target=\"_blank\" rel=\"noreferrer noopener\">HumanTrak<\/a>, odnosno (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35320148\/\" target=\"_blank\" rel=\"noreferrer noopener\">Collings i dr., 2022.<\/a>). <a href=\"#references\" title=\"\">[1]<\/a> Zabilje\u017eene su ozljede prednjeg kri\u017enog ligamenta bez kontakta koje su se dogodile u sljede\u0107ih 18 mjeseci kako bismo mogli poku\u0161ati utvrditi po \u010demu se igra\u010di koji su zadobili ozljedu razlikuju od onih koji je nisu zadobili.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img decoding=\"async\" width=\"446\" height=\"433\" src=\"https:\/\/www.ultrax.ai\/wp-content\/uploads\/2023\/11\/02.png\" alt=\"\" class=\"wp-image-1793\" srcset=\"https:\/\/www.ultrax.ai\/wp-content\/uploads\/2023\/11\/02.png 446w, https:\/\/www.ultrax.ai\/wp-content\/uploads\/2023\/11\/02-300x291.png 300w\" sizes=\"(max-width: 446px) 100vw, 446px\" \/><figcaption class=\"wp-element-caption\">Slika 2. Kinematika trostrukog vertikalnog skoka na jednoj nozi kori\u0161tenjem sustava za snimanje pokreta bez markera (HumanTrak).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">\u0160to smo prona\u0161li?<\/h2>\n\n\n\n<p class=\"\">Rezultati su otkrili da je niz \u010dimbenika povezan s pove\u0107anim rizikom budu\u0107ih ozljeda prednjeg kri\u017enog ligamenta bez kontakta, a to su:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\">ni\u017ei omjer snage aduktora i abduktora kuka,<\/li>\n\n\n\n<li class=\"\">ve\u0107i vertikalni posko\u010dni dinami\u010dki valgus koljena i ipsilateralni kutovi fleksije trupa,<\/li>\n\n\n\n<li class=\"\">ve\u0107e vr\u0161ne sile polijetanja CMJ-a i,<\/li>\n\n\n\n<li class=\"\">prethodna ozljeda prednjeg kri\u017enog ligamenta (ACL).<\/li>\n<\/ul>\n\n\n\n<p class=\"\">Normativne podatke za sve na\u0161e rezultate ispitivanja mo\u017eete prona\u0107i <a href=\"https:\/\/tcollings.shinyapps.io\/normative-data-app\/\" target=\"_blank\" rel=\"noreferrer noopener\">ovdje<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Snaga kuka<\/h2>\n\n\n\n<p class=\"\">Igra\u010di s ja\u010dim abduktorima kuka u odnosu na aduktore imali su pove\u0107an rizik od budu\u0107ih ozljeda prednjeg kri\u017enog ligamenta (1,97x ve\u0107a vjerojatnost ozljede za svako smanjenje izometrijskog omjera snage aduktora i abduktora od 14%).<\/p>\n\n\n\n<p class=\"\">Mehanizam(i) koji obja\u0161njavaju ovu povezanost nisu jasni, me\u0111utim, neravnote\u017ea izme\u0111u ove dvije mi\u0161i\u0107ne skupine mo\u017ee utjecati na koordinaciju zgloba kuka tijekom visokorizi\u010dnih zadataka poput skakanja i promjene smjera.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Kinematika trostrukog vertikalnog skoka na jednoj nozi<\/h2>\n\n\n\n<p class=\"\">Doskok s ve\u0107im dinami\u010dkim valgusom koljena i ipsilateralnim kutovima fleksije trupa (tj. prema nozi na kojoj se stoji) zna\u010dajno je pove\u0107ao rizik od naknadne ozljede prednjeg kri\u017enog ligamenta. Ipsilateralna fleksija trupa pove\u0107ava moment abdukcije koljena pove\u0107anjem kraka momenta trupa u odnosu na koljenski zglob, \u0161to tako\u0111er mo\u017ee pove\u0107ati optere\u0107enja ACL-a.<\/p>\n\n\n\n<p class=\"\">Ovi podaci upu\u0107uju na to da strategije usmjerene na smanjenje dinami\u010dkog valgusa koljena i lateralne fleksije trupa pri doskoku na jednu nogu mogu smanjiti rizik od budu\u0107ih ozljeda prednjeg kri\u017enog ligamenta.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">CMJ kinetika<\/h2>\n\n\n\n<p class=\"\">Igra\u010di koji su proizveli ve\u0107e vr\u0161ne sile odraza na prednji kri\u017eni ligament (CMJ) imali su zna\u010dajno ve\u0107i rizik od naknadne ozljede prednjeg kri\u017enog ligamenta (ACL) od onih koji su proizveli ni\u017ee sile. Predvi\u0111ene vjerojatnosti izvedene su iz univarijabilnih (neprilago\u0111enih) logisti\u010dkih regresijskih modela. Isprekidana horizontalna linija ozna\u010dava osnovni rizik (5,4%), a presjek s predvi\u0111enom vjerojatno\u0161\u0107u ozna\u010dava vrijednost pri kojoj se rizik od ozljede ACL-a pove\u0107ava\/smanjuje u odnosu na osnovni rizik. Vrijednosti koje odgovaraju predvi\u0111enim vjerojatnostima ozljede ACL-a za 2%, 5,4%, 10%, 15% i 20% ozna\u010dene su na krivulji crnim to\u010dkama. Slika je replicirana iz Collingsa i suradnika, 2022. <a href=\"#references\" title=\"\">[2]<\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Kombiniranjem terenskih mjera pobolj\u0161ava se procjena rizika od ozljeda<\/h2>\n\n\n\n<p class=\"\">Kada se kombiniraju u multivarijantnom klini\u010dkom prediktivnom modelu, na\u0161a baterija probira temeljena na terenu ispravno je identificirala gotovo 4 od 5 igra\u010da koji su zadobili naknadnu ozljedu prednjeg kri\u017enog ligamenta. <\/p>\n\n\n\n<p class=\"\">Me\u0111utim, pojedina\u010dne varijable same po sebi bile su samo neznatno bolje u predvi\u0111anju ozljede od bacanja nov\u010di\u0107a. Ovo je va\u017ena stvar koju treba razumjeti pri procjeni rizika od ozljede igra\u010da.<\/p>\n\n\n\n<p class=\"\">Pojedina\u010dne varijable su lo\u0161i pokazatelji rizika od ozljeda jer ne mogu obuhvatiti slo\u017eenost ozljede i objasniti velike varijacije me\u0111u pojedincima. Mnogo to\u010dnije procjene rizika od ozljeda zahtijevaju provo\u0111enje niza testiranja i kombiniranje va\u017enih varijabli u multivarijabilni model. Me\u0111utim, to se mo\u017ee posti\u0107i samo s velikim skupovima podataka o pojedincima, zajedno s njihovim zapisima o ozljedama. Razvoj i validacija takvih modela uzbudljivo je podru\u010dje istra\u017eivanja koje se zatim mo\u017ee primijeniti u klini\u010dkoj praksi.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">\u0160to je s asimetrijama izme\u0111u udova u snazi i biomehanici?<\/h2>\n\n\n\n<p class=\"\">Nismo prona\u0161li uvjerljive dokaze da su asimetrije izme\u0111u nogu u snazi i biomehanici povezane s rizikom od ozljede prednjeg kri\u017enog ligamenta (ACL). Naknadno ozlije\u0111eni i neozlije\u0111eni igra\u010di pokazali su asimetrije 10-20% u snazi donjih udova i asimetrije 10-50% u varijablama sile reakcije tla CMJ-a, \u0161to sugerira da je odre\u0111eni stupanj asimetrije \u201enormalan\u201c, barem kod elitnih nogometa\u0161ica.<\/p>\n\n\n\n<p class=\"\">Zanimljivo je da smo primijetili da je \u201eneusmjerena\u201c asimetrija u snazi ekscentri\u010dnog fleksora koljena na NordBordu bila povezana s pove\u0107anim rizikom od naknadne ozljede ACL-a, me\u0111utim, ja\u010da noga je bila ozlije\u0111ena jednako \u010desto kao i slabija noga. Me\u0111utim, djevojke i \u017eene i dalje se suo\u010davaju s mnogim preprekama za sudjelovanje u sportu i postizanje visokih rezultata, uklju\u010duju\u0107i neprihvatljivo visoke stope ozljeda prednjeg kri\u017enog ligamenta.<\/p>\n\n\n\n<figure class=\"wp-block-pullquote\"><blockquote><p><strong>Djevojke i \u017eene se i dalje suo\u010davaju s mnogim preprekama u sudjelovanju u sportu i postizanju visokih rezultata, uklju\u010duju\u0107i neprihvatljivo visoke stope ozljeda prednjeg kri\u017enog ligamenta (ACL).<\/strong><\/p><\/blockquote><\/figure>\n\n\n\n<p class=\"\">Unato\u010d zna\u010dajnim istra\u017eiva\u010dkim naporima tijekom nekoliko desetlje\u0107a, strategije prevencije ozljeda prednjeg kri\u017enog ligamenta (ACL) jo\u0161 uvijek su pod velikim utjecajem klini\u010dkih pretpostavki i mi\u0161ljenja stru\u010dnjaka, a ne empirijskih dokaza.
